Innovative State of Mind, LLC is committed to protecting your health information. Innovative State of Mind is required by law to maintain the privacy of Protected Health Information (PHI). PHI includes any identifiable information that we obtain from you or others that relate to your physical or mental health, the health care you have received, or payment for health care. As required by law, this disclosure provides you with information about your rights and our legal duties and privacy practices with respect to the privacy of PHI. In order to provide treatment or to pay for your healthcare, Innovative State of Mind will ask for certain health information. This health information will be put into your personal record. The record may contain your diagnosis, assessments, symptoms, test results, and treatment. That information, referred to as your health or medical record, and legally regulated as health information, may be used for a variety of purposes. Our HIPAA-compliant software verifies that the physical and technical safeguards we implement satisfy the requirements outlined in the HIPAA Privacy Rule and HIPAA Security Rule. Access controls ensure that users only have access to the information necessary to complete their job duties, keeping your client’s records secure. CONFIDENTIALITY

The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.

Exceptions include:

• Suspected child abuse or dependant adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
• If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
• If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
