Client demographics, to include name, contact phone number,  email address may be requested orally, verbally, or through this website to discuss potential services and input into secured electronic medical records in which clients are scheduled for an appointment. 

Payment

Innovative State of Mind, LLC accepts cash, check, credit cards and insurance benefits as payment.  Your personal health information may be required for reimbursement of treatment and/or payment for health care services rendered. Credit cards may be securely stored in electronic medical records.  This website does not request or accept credit card information or payments. Innovative State of Mind, LLC has achieved and is adherent to the Payment Card Industry Data Security Standards and is PCI Compliant.

Limiting Use / Disclosure

Client confidentiality is of utmost importance. The law protects the relationship between a client and psychotherapist with the exception of suspected child/dependent/elderly abuse or neglect, threatening serious bodily harm to another person(s), or if a client intends to harm themselves. Innovative State of Mind, LLC may also be required to share information without consent as required by the law when enacted by a subpoena or of public health concerns. Additionally, the use of disclosure without consent may be necessary for treatment or payment in order to bill and collect payment for your health care services and/or to determine your eligibility to participate in our services. Other examples of disclosure may include notification to family that is involved in your direct care or listed as an emergency contact. User access to your health information is protected and only granted to staff that is necessary to complete their job duties. This may include notification to you for scheduling and/or treatment recommendations.

Accuracy

Client information will be maintained securely and updated annually on Innovative State of Mind electronic medical records only.

Security of Information

Innovative State of Mind, LLC is committed to protecting the information we receive from you. We take appropriate security measures to protect your information against unauthorized access to or unauthorized alteration, disclosure or destruction of data. To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we maintain appropriate physical, electronic, and managerial procedures to safeguard and secure the information and data stored on our system. Our HIPAA compliant electronic medical records verifies that the physical and technical safeguards implemented satisfy the requirements as outlined in the HIPAA Privacy Rule and HIPAA Security Rule. Access controls to medical records are limited to users that are necessary to complete their job duties. While no computer system is completely secure, we believe the measures we have implemented reduce the likelihood of security problems to a level appropriate to the type of data involved. 

Client Access

Upon request, a client shall be informed of the existence, use and disclosure of their information, and shall be given access to it, with the exception of psychotherapy notes as protected by the law. Clients may verify the accuracy and completeness of their information, and may request that it be amended or removed, if appropriate. All requests must be completed in writing.
